Three shifts I watch closely after 50

Gums. Early gum disease often does not hurt. Bleeding when you brush, puffy edges, breath that will not clear, or teeth that look longer are signals I take seriously. We chart gums at cleanings. Gingivitis can often reverse with professional cleaning and home care. Once periodontitis starts, lost bone does not grow back, but treatment can stop the damage. When deeper gum or implant care belongs in the plan, our periodontist, Dr. Shah, sees patients here at mend, so you are not sent across town with a new chart.

Dry mouth and medications. Many common medicines reduce saliva. Less saliva raises cavity risk in new places, especially along exposed roots. Bring your medication list. I may recommend fluoride for roots and a different cleaning interval based on risk, not a one-size schedule.

Wear, cracks, and missing teeth. Crowns, bridges, and implants are tools when they fit your goals and bone support. I explain tradeoffs and cost before any commitment. No pressure to replace a tooth on the first conversation if you are still deciding.

What I include in a checkup for older adults

• Exam with imaging when it adds information
• Gum charting and a maintenance plan if you have had perio treatment
• Oral cancer screening at every exam
• Root sensitivity and fluoride discussion when roots are exposed
• A plain-language plan you can accept, adjust, or decline

If I recommend cleanings every three to four months after periodontal treatment, that is because bacteria return below the gumline faster in treated mouths. It is a clinical schedule, not a sales cadence.
